Absentee Form

The St. Mary鈥檚 School App is FREE at the  or  and can be downloaded to your Android or iPhone. Once downloaded you can: 

  • Fill out an Absent Form that will be emailed directly to your child鈥檚 teacher and the office. (This eliminates calling the school office)
  • View our school calendar
  • Email your child鈥檚 teacher directly
  • Instant access to St. Mary鈥檚 Social Media: Facebook, Instagram and Twitter accounts 

Alternatively you can send an email to our office from your desktop or mobile device. Please include the full name of the student, their grade, and the reason for the absence.

If you still prefer pen and paper, you can download the form below, print it, complete it, and then send it with your child upon his or her return.
